Rivetus has been serving the geriatric Post-Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians. Our perspective is unique in providing continuity of care across the health care continuum in the Skilled Nursing, Home Health, and Outpatient settings. This position demonstrates the commitment Rivetus has for its clinicians to stay engaged with their patients throughout the recovery process as it emphasizes Home Health services with a portion of time in the Skilled Nursing Facility allowing the clinician and patient to build trust and familiarity with one another.

General Summary:
In this role you will provide physical therapy services to patients impaired by physical dysfunction, including assessment, treatment planning and implementation of treatment plans. You’ll be a team member of the Rivetus Take Your Therapist Home program, that emphasizes continuity of care by providing Home Health visits after a SNF stay. Completes appropriate documentation and adheres to policies, procedures and current standards of clinical practice. Provides supervision for assistants as assigned.
Facility Location: 7025 Liley Road, Canton, MI. This is a hybrid role providing Home Health therapy services in conjunction with SNF based therapy services. Home Health geographic area is the vicinity around our Canton facility. Home care services are paid per visit, and SNF services are paid at an hourly rate.

Monday - Friday, Full Time
Responsibilities:
  • Conducts physical therapy evaluations to determine patient physical status using standardized tests, and measures to assess patient’s strength, motor development, joint mobility, sensory perception, balance, functional capacity, circulatory efficiency, and tolerance to physical activities.
  • Develops and implements physical therapy treatment plans using strengthening, modalities and retraining strategies consistent with the professional and ethical standards of the APTA in order to improve and maintain function and to reach patients’ highest potential for independence.
  • Checks, instructs, assigns and reviews the work of PTAs and technicians as assigned.
  • Assess effects of treatment on an ongoing basis and modifying the patient’s plan to achieve maximum benefit.
  • Evaluates, fits and adjusts prosthetic/orthotic devices along with prosthetist/orthotist.
  • Confers with physician and other practitioners to obtain additional patient information, suggest revision in treatment program and integrate physical therapy treatment with other aspects of patient’s health care.
  • Provides documentation to maintain quality of the medical record as it relates to accuracy, continuity of care, reimbursement guidelines, and agency requirements.
  • Participates, as requested, in patient and family care conferences.
  • Educate and instructs patient and family/caregiver(s) in treatment procedures to be continued when not in therapy or as a home exercise program.
  • Participates in Quality Assurance activities as directed by Agency Administrator, and/or site-specific Director of Therapy.
  • Promotes current standards of clinical practice through participation in internal and external continuing education, in-service programs and certifications sponsored by the agency.
  • Participates in new program development to expand the clinical services of the rehabilitation agency.
Required Qualifications:
  • Successful completion of an accredited Physical Therapy program.
  • Licensure as required by the State of Michigan.
  • Rotates to other clinic locations to meet patient care needs and other duties as assigned.
  • Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
  • Ability to react effectively to the needs of patients and their families.
  • Physical Ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
