Description

The TOC LPN Care Manager is a core member of the patient care team. The TOC LPN Care Manager is responsible for supporting patients between hospitals, acute care facilities, and home care settings. This role will support patients as they navigate transitions of care. The care manager will identify discharged patients, coordinate post discharge appointments, obtain information, and support the patient through transition from hospital to home. The TOC LPN collaborates with providers and center staff to close gaps in care, address barriers to care, and support self-management. This professional has a well-developed understanding of community resources and can multi-task effectively while providing personalized, compassionate care.

Requirements

Completion of State approved course of study for Practical Nurse training. Must maintain knowledge of latest developments in nursing practice techniques.

· Current PA Licensed Practical Nurse licensure.

· Maintains current Cardiopulmonary Resuscitation certification (CPR).
