River Edge Behavioral Health is Hiring a D186 - Jail Services - Discharge Planner / Re-Entry Specialist Near Macon, GA

River Edge is currently seeking a Full-time Discharge Planner/Re-entry Specialist for the Judiciary In-reach Program. Role Summary Under the supervision of the Program Manager, the Discharge Planner/Re-entry Specialist will assist detainees in the Bibb County Sheriff's Office (BCSO) with navigating community resources and services as they reestablish their lives in the community. The Discharge Planner's goal is to educate and advocate with individuals to prepare them and their families for a successful release from jail. The discharge planner collaborates with the multi- disciplinary team about what the ongoing treatment needs are and how these needs can be addressed in the community.Duties and ResponsibilitiesComplete required documentation in order to support skill training, needs assessments as assigned, and resource development for individuals.Coordinates with jail behavioral health staff to provide highly coordinated support for detainees, transitioning to the community.Regularly Attend any community events to build partnerships.Coordinates teamwork, resource acquisition and service planning needed to enable expedited release from BCSO and coordinates linkage to initial post-release resources.Reviewing re-entry and individual goalsCoordinates program-related data collection and analysis, outcomes measurement, service evaluation and improvement for reporting to River Edge Quality Improvement Committee and funder.Support linkage to mental health/substance abuse treatment, stable medication management, housing, employment, and assisting with benefit applications.Monitor wellness check-ins with mentors.Support Group Facilitators in the jailQualificationsBachelor's degree in social work, Sociology, Criminal Justice, and/or Psychology.At least one (1) year experience working with behavioral health clients (mental health, substance abuse, developmental disabilities).Must have strong clinical skills.Must have ability to multi-task and remain organized.All applicants must pass a satisfactory background clearance and pre-employment drug test.Provide 2 Professional references.Additional Information: The Application ProcessAll qualified applicants will be considered.This position is subject to close once a satisfactory candidate pool has been identified.The hiring managers will contact only those selected for an interview.Applicants who are not selected will receive notification via email.Due to the volume of applications received, we are unable to provide information on application status by phone or email.Safe Working EnvironmentWe at River Edge believe every employee has a right to a safe work environment.
