Rising Ground is Hiring a Bilingual Case Planner - Brooklyn Near Brooklyn, NY

Rising Ground is looking for a person who is fluent in speaking and writing in English and Spanish with experience in child welfare or intern in a child welfare setting. Must have your bachelor's in social work or related field.

Family Support Program is looking for visionaries eager to utilize an evidence-informed model to support families to navigate, survive, and thrive in their worlds. Join us in this ground-breaking initiative to provide an evidence-informed intervention called Solution Based Casework (SBC). In addition, we provide to families case management services, advocacy, referrals, assistance with childcare, housing, income support, food pantries, and medical and mental health services.

The Solution Based Casework (SBC) Model prioritizes working in partnership with families, focuses on pragmatic solutions to difficult situations, and celebrates family successes.

Through the SBC Model Case planners are to help strengthen families, stabilize, and maintain family functioning, while continually assessing for risk and safety issues. Services are provided to at-risk families referred by the Administration for Children’s Services or self-referred.

Responsibilities

  • Case Planner will have a caseload of 12 families. Case Planners will perform assessments of a child or children and family needs; implementing preventive services to avert, when possible, foster care placement and disruption of the family unit. Work includes counseling families toward resolving individual and family level concerns, outcomes, and developing family led action plans. This is facilitated by utilizing agency and community resources to assist families in achieving and maintaining self-sufficiency and independence. This work may include non-traditional work hours; Case Planners must make themselves available to meet based on the family’s availability. Furthermore, Case Planners are provided laptops and cell phones which allows staff to work remotely and not travel to the office daily.
  • Conducts case work contact visits 2X per month; in the home; in the community; and/or at school. Perform face to face counseling sessions as mandated and as appropriate, based on need. Record and document assessment results, case plans, services provided, visits made and progress towards goals in database systems (CONNECTIONS and PROMIS). Ensures that case management activities, documents, record-keeping, and reporting comply with regulatory guidelines and quality standards in a timely manner
  • Provides and arranges for appropriate referrals; community based, and other services related to the needs (i.e., social, and emotional, therapeutic services, medical, educational, vocational, financial, legal, childcare, home management, entitlement advocacy, substance abuse treatment, anger management, and parenting skill training) for the child/ren and family. Monitors treatment activities and levels of participation and effectiveness; advocates for additional needed services.
  • Promptly addresses all safety and protection needs of the child; ensure that living environments are safe. Investigates and responds to all reports of observation of suspected neglect, abuse or exploitation of children; immediately reports and address unsafe or reasonably suspicious situations consistent with actions of Mandated Reporters; takes precautions to eliminate and reduce risk and safety issues. It is the organizations’ expectation of case planners to maintain professional and technical knowledge.
  • Participates as a responsible, cooperative member of the interdisciplinary team; contributes to the team efforts; maintains positive relationships with the Edwin Gould/Rising Ground constituency, i.e., children, families, caretakers, representatives of governing agencies, colleagues, service providers, and the community; interprets the program to service providers and community, assist in developing community collaboratives.
  • Work is performed under the supervision of an administrative manager, usually a Preventive Supervisor. Close supervision is received in the form of review of case work plans, one-on-one supervision, case work conferences, and case presentations.

Qualifications

  • Bachelor’s degree in social work or related field from an accredited college or university required.
  • Experience in children and families in a Human Service setting and/or Child Welfare System
  • Familiarity with using PROMIS and Connections.
  • Computer skills in Microsoft Office suite and excellent writing skills are required.
  • Bilingual is a plus. (Spanish, Haitian/Creole)
