Rise home health agency is currently seeking a Registered Nurse to join our team. In this role, you will play a crucial part in providing high-quality care to our clients by conducting initial and ongoing comprehensive assessments of their needs, including Outcome and Assessment Information Set (OASIS) assessments, and reevaluating at appropriate intervals.

Responsibilities:

• Collaborate with healthcare professionals to coordinate total patient care, ensuring comprehensive health assessments, continuous monitoring of patient conditions, and the coordination of services and training activities.
• Consult with attending physicians to provide optimal patient care.
• Evaluate the effectiveness of nursing services delivered to both patients and their families.
• Provide supervision and training to LPNs and HHA’s to enhance their skills and ensure the highest standards of care.

Qualifications:

• Proven experience as a Registered Nurse.
• One year or more of experience in a Home Healthcare setting.
• Familiarity with Medicare and Medicaid guidelines.
• Ability to contribute to the quality of care rendered through effective communication with nursing management and staff.
