Nurse Care Manager in Plymouth

Salary:$28.00-$33.00 an hour

Shift Type: per diem

In this role, we stand prepared to equip you with the right personal protective equipment (PPE) and are ready to assist as you care for our vulnerable population of seniors and adults with disabilities.
Nurse Care Manager Must Haves:
  • Graduate of an accredited practical nursing program and licensed to practice in the state
  • 1-3 years of nursing experience (home health/hospice preferred)
  • Staff License Practical Nurse (LPN) and/or Registered Nurse (RN) must possess and maintain a current CPR certification
  • Knowledge, ability, and the necessary skill and judgment to perform all nursing procedures as outlined in the scope of practice in the State’s Nurse Practice Act and under the direction of the registered nurse and physician orders
  • Staff License Practical Nurse (LPN) and/or Registered Nurse (RN) must have a valid driver’s license and use an insured automobile for work or access to adequate transportation
  • Available for work on weekends and holidays as required
  • Staff License Practical Nurse (LPN) and/or Registered Nurse (RN) must be able to read, write, speak, and understand English as needed in the job
  • Participate in in-service education programs as mandated by State law (this requirement need not be met exclusively through Right at Home-sponsored in-service programs)
  • Must be able to satisfy the Massachusetts Covid-19 Vaccination mandate for Home Care
WORKING ENVIRONMENT
  • Primarily works from home on call; limited travel may be required to clients' homes.
