Care Coordinator

If so, Right at Home has the opportunity for you! The Care Coordinator is responsible for creating extraordinary client and family experiences and ultimately manages best in home care care plans.

Responsibilities :

• Care Coordinators develop and maintain the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources.
• Provide supervisory and quality visits with clients and caregivers
• Plays an integral role in interfacing with clients and families to ensure safe, appropriate and meaningful care is being provided.
• Serves as a mentor to caregivers and supports their care delivery.
• Maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment.
• Play the integral role in helping Right at Home improve the lives of as many people as possible in our community.
• Increases care hours, develops a profitable referral source pipeline, and gains market share.
• Pre-plans weekly sales activities, creates opportunities for communication with referral sources, and maintains all sales activity in a Customer Relationship Manager database
