Primary Care Clinic Nurse:

The Clear Path Clinic, S.C. in conjunction with the Rice Lake Area School District is seeking to fill a nursing position for an employer based Primary Care Clinic supporting the primary care needs of the staff and families of the Rice Lake Area School District and Henry Repeating Arms. This position will, in conjunction with a Medical assistant, support a doctor and a Nurse Practitioner, in caring for this population full-time (30 hours/week). The goal is to provide the best possible patient focused care possible. This style of clinical practice, which is often referred to as Advanced Primary Care or as a Direct Primary Care practice, is designed to act as the medical home for the patients that it services. In this setting the clinic staff are able to take the time to support the needs of the patient to the fullest extent possible. The expectation for the clinic is that the providers care for the patients as if they were their own family members. The practice cares for over 1200 patients and is open to patients 43 hours a week. There are no on-call requirements for this position. The position itself will support a primary care practice providing primary care nurse support, care management, and health coaching support.

Qualifications include: Graduate of an accredited school of nursing, RN licensure and CPR certification.
