A Nurse Case Manager (RN) will service in accordance with established policies, procedures and protocols of the healthcare organization.

Compensation:

• $65/Visit
• $85/Start of care

Requirements:

· Graduate of an accredited Nursing Program and Valid RN license in IL

· Valid Driver’s license, proof of insurance, and dependable vehicle

· Minimum of one year nursing experience required; hospital, home health, and Oasis documentation preferred

· Adherence to scheduled days and hours of work based on workload assigned

· Knowledge of state and federal home health regulations

· Clinical proficiency in nursing specialty, strong communication skills and ability to work with minimal supervision

· Educate patients/family on their plan of care, established goals, and self-care

· Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing

License/Certification:

· Driver's License (Required)

· RN (Required)

· BLS (Required)

Responsibilities:

· Provides direct patient care, develops, and implements an individualized plan of care, as defined by IL guidelines, in coordination with other members of the health care team.

· Submits accurate, complete, and timely documentation, per policy.

· Travel to patients wherever they reside: Home, nursing facility etc.

· Coordinates with the interdisciplinary team and other individuals in the patient’s care plan.

· Wound care, IV management, PICC Line care, G-Tube care, Foley care

· Performs biweekly LPNs and CNAs supervisory visits

· Attends mandatory informational/education/staff meetings.

8:30am-5pm
