Retreat Behavioral Health is excited to announce the expansion of our programming and we are looking for a dynamic, internally motivated, professional to join the clinical team as a Patient Care Coordinator.
POSITION SUMMARY
A well-organized and highly motivated individual that can obtain and analyze information in a collaborative method as it pertains to a patients treatment in the areas of, but not limited to, assessment, evaluation, planning, financial counseling, medical records, and integration of care. Patient care coordination is a collaborative process to meet an individual’s and family’s comprehensive health needs through communication and available resources, to promote quality care.
QUALIFICATIONS
Proficiency in the skill of reading and writing and communication are essential as well as one’s ability to work in a collaborative atmosphere with colleagues, business professionals, families, and patients. Knowledge of clinical assessment methods, individual and group therapy techniques, as well as knowledge of residential treatment programs is required. An effective Care Coordinator will have an understanding of human behavior and motivational interviewing to assist in facilitating change. A comprehensive knowledge of Microsoft and the EMR is essential for the production of appropriate documentation to provide necessary information as it pertains to tailoring clinical resources to satisfy the needs of the patient. Time management and organization skills will be considered professional strengths of the Care Coordinator for the purpose of completing their responsibilities and treatment needs of the patient efficiently. The initial assessment of these qualifications will be conducted during pre-hire interview by senior management staff members.
Preferred – A Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and I year of clinical experience (minimum of 1820 hours) in a health or human services agency, preferably in a drug and alcohol setting. If a person’s experience did not take place in a drug and alcohol setting, the individual’s written training plan will specifically address a plan to achieve counseling competency in chemical dependency issues.
OR
An Associate’s Degree from an from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 2 years of clinical experience (minimum of 3640 hours) in a health or human service agency, preferably in a drug and alcohol setting. If a person’s experience did not take place in a drug and alcohol setting, the individual’s written training plan will specifically address a plan to achieve counseling competency in chemical dependency issues.
OR
A High School diploma or equivalent degree from an accredited institution with 6 months of experience in social work, or case management position or clinical experience in a health of human services agency, preferably in a drug and alcohol setting. If a person’s experience did not take place in a drug and alcohol setting, the individual’s written training plan will specifically address a plan to achieve counseling competency in chemical dependency issues
