Description

Job Overview: The SSVF Case Manager will carry a caseload up to 25 clients, which is subject to change due to client demand; and work closely with the local Veterans Association Health Care for Homeless Veteran (HCHV) program, the local Veterans Center and local social services providers and benefit agencies, shelters and other community agencies to identify and engage veterans and veteran families that are literally homeless or at imminent risk of homelessness. The Case Manager/Outreach Specialist will screen all outreach contacts for eligibility for reStart’s SSVF program. This position will be a part of coordinated entry weekly housing placement meetings and community case conferencing based on the by-name registry. Also, community/street outreach is to be strategically planned and conducted monthly.

Responsibilities/Duties:

  • Models positive and healthy relationship building by treating all participants, staff and volunteers equitably with caring, kindness, respect, safety and dignity
  • Follows all policies, procedures, code of conduct and attendance rules of the organization
  • Maintains strict confidentiality of all information and adheres to the policies in the use of computer technology and all other devices
  • Support SSVF program participants with customized housing to meet client needed
  • Screen all outreach clients for eligibility for the SSVF program using the approved Eligibility Screening tool.
  • Assist in the selection, evaluation, and supervision of clients who qualify for SSVF services.
  • Collaborate with team members to promote client progress and address challenges with daily effective communication through communicate emails
  • Conduct office, home and field visits to program landlords to ensure success
  • Coordinate continuity of services with collateral agencies and coordinate services in the community.
  • Provide referrals to ESG programs, HUD-VASH programs, Shelter Plus or other community resources, as appropriate for contacts who are not eligible for SSVF or who require longer-term housing stabilization resources.
  • Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with SSVF guidelines
  • Carry a caseload up to 25 clients to ensure stability is maintained; Caseload subject to change due to client demand at anyting
  • Facilitates client intake process and compiles client files ensuring all program eligibility documentation is complete
  • Collaborates with clients to complete Housing Stability Plan (HSP) ensuring the veteran is aware of their participation requirements and goals
  • Conducts home visits to provide case management services and ensure health and welfare of clients
  • Assists in quality management activities, data collection, and preparing reports
  • Completes accurate and timely data entry into the HMIS system, as applicable; and other required internal and external databases
  • Documents client referrals to external services
  • Focuses on providing case management services that result in clients transitioning to permanent housing and retention effects
  • Learns and maintains knowledge of community employment agencies and procedures, along with any modifications to those procedures.
  • Works with clients to develop an Individual Employment Development Plan; assesses, alleviates, and reduces any barriers to employment; assists the veteran with the planning of short-term and long-term employment goals to include education and training goals
  • Provides individualized assistance to clients in developing cover letters, resumes and obtaining other supportive services.
  • Provides customize case management services to family members in the veteran household
  • Ensures that dependent children are enrolled in school and connected to the appropriate services within the community, including early childhood education programs
  • Maintains compliance with VA regulations and organization protocols in requesting and distributing Temporary Financial Assistance to third-parties on behalf of veteran households to include cross-referencing HMIS
  • Conduct Rent Reasonableness and ensure habitability standards are met for veteran families rental units
  • Coordinate access to obtain required documentation during COVID-19
  • Assist with coordination of local food pantries and health center during COVID-19
  • Assist with transportation
  • Monitor the hotline daily to conduct screenings
  • Role models effective team behavior; Substitutes for other staff when need arises
  • Demonstrates effective communication skills in building relationships with all employees and participants
  • Creates positive working relationships with community agencies and service providers to facilitate the access to area resources for participants
  • Other duties as assigned

Qualifications


Qualifications:

  • Knowledge of Missouri and Kansas Fair Housing and Reasonable Accommodations
  • Ability to work with a variety of landlords and property management companies
  • Ability to maintain and execute confidential information according to HIPAA standards
  • Ability to coordinate multiple projects simultaneously in a high-pressure and time sensitive environment
  • A high level of tolerance and understanding of individuals who require services with urgent multiple case management and health needs
  • Ability to work independently and within team environment and exercise mature judgment
  • Willingness and ability to regularly provide community and in-home based services
  • Strong written and verbal communication skills
  • Veteran (preferred).
  • Bachelor’s Degree in human services or a related field and two years of experience
  • Five years cumulative experience working within social services and populations in crisis including the homeless population and people with disabilities.
  • Education and/or experience with property management and realty functions.
  • Experience working with homeless and/or veterans preferred.
  • Strong oral and written communication skills.
  • Professional manner and appearance.
  • Ability to take direction.
  • Unique understanding of working with homeless individuals/families, veterans and/or other underserved populations with three years minimum experience required.
  • MS Office experience, especially Outlook
  • Must pass a criminal records check and pre-employment drug screening
  • Must have valid driver’s license and current automobile insurance and reliable transportation
  • Ability to obtain CPR/ First Aid certification
