POSITION SUMMARY:
As the Hospice Licensed Practical Nurse (LPN), you will have the ability to get to know your individual patients, provide meaningful education and support in executing tailored Care Plans. You will provide hospice care to patients in their own environment. Become an integral part of a dynamic, multi-disciplinary teams while maintaining a high level of autonomy.
