They are seeking a talented Home Health Licensed Practical Nurse (LPN) for 37.5 hours per week.

POSITION SUMMARY:

As the Hospice Licensed Practical Nurse (LPN), you will have the ability to get to know your individual patients, provide meaningful education and support in executing tailored Care Plans. You will provide hospice care to patients in their own environment. Become an integral part of a dynamic, multi-disciplinary teams while maintaining a high level of autonomy. QUALIFICATIONS:

• Hospice experience (strongly preferred)
• 2-3 years of med/surg nursing experience if no previous Hospice experience (required)
• Licensure: State of New Hampshire or Compact State Nursing License
• NH Driver’s License and current automobile and liability insurance
• Tech savvy (preferred)
• Strong documentation, communication, and symptom management skills (required)
• Must be flexible and family oriented
• Current BLS

Job Type: Full-time

Hours per week:

• 37.5

Schedule:

• Monday -Friday; 8a-4:30p
• Occasional weekend and on-call requirements

Compensation:

• $25 - $35 Per/Hour
• On-call and Call-back compensated
• Mileage reimbursement at IRS rate

License:

• Licensed LPN (required)
• Licensed in the State of New Hampshire or Active Compact (preferred)

Work Location:

• Out in the field providing patient visits (home)

Coverage Area:

• 30-mile radius of Meredith

Productivity Requirements:

• 4-5 visits per day
