RN Case Managers oversee the entire care of the patient beginning to end while under the care of Residence Home Care.

· Provide care wherever the patient calls home.

· Manage other disciplines such as LPNs, aides and therapy staff to give the best care needed for each patient.

· Ability to complete OASIS, recert, d/c, ROCs documentations within an EMR System. (WellSKY)

· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.

Our RN Case Managers participate in a flexible on-call rotation with additional compensative. We also have a dedicated weekend team!

· Attend meetings, CEUs in the North Olmsted, Ohio office on a regular basis

Requirements:

Working for a home health care agency requires the following:

· Current RN Ohio license in good standing.

· Reliable auto and current auto insurance

· Current CPR.

· Solid understanding of Medicare/Medicaid guidelines as it applies to Home Health Care.

· Home Health Care experience is preferred
