Position Overview Under the supervision of the Manager of Team Care Coordination the Returning Citizen Community Reentry Program Case Manager provides direct case management services to homeless individuals, especially individuals in the re-entry population, with a history of criminal justice involvement. The Case Manager works towards the goal of moving clients quickly to an appropriate housing opportunity while addressing their specific needs, including substance abuse treatment, mental healthcare, physical healthcare, identification, and income development.Qualifications For Position Education: Bachelor’s degree required, Master’s in Social Work or related field preferredExperience: Minimum of three (3) years experience in case management. Rescue Mission of Trenton requires all new employees to provide proof that they are fully vaccinated and have received a booster (where eligible) against COVID-19 prior to start of employment, unless the Rescue Mission of Trenton has granted the employee a medical or religious exemption. Employees who are not eligible for a booster at the time of an offer of employment provide proof they have received a booster upon eligibility and upload proof of same. Failure to provide proof of primary vaccination and booster will result in recession of a candidate’s offer of employment or disciplinary action up to and including termination.
