Under the direction of the Case Manager the Assistant Case Manager will provide comprehensive case management services to individuals with intellectual Disabilities/Autism in the agency’s Meaningful Day, Personal Supports, and Respite programs.
Education/Training/Experience Requirements:
Skills
Knowledge of COMAR 10.22.11 Developmental Disabilities Administration
Participant intake and assessment
Person Centered planning development for participants served
Quarterly progress reports
Behavior Support services and data collection
Generate and monitor participant Outcome/Goals
Staff training on Specific Needs and ongoing development trainings
Complete on-site supervision and monitoring of all participant activities and services
Quarterly staff evaluations and reviews
Ensure Meaningful day site is in compliance with COMAR regulations

Conflict Management & Problem-Solving skills

Excellent written/oral communication skills
Basic computer knowledge

Valid driver’s license and automobile insurance
CPR/First Aid
