```Job Summary:```
We are seeking a skilled and compassionate Nurse Practitioner to join our healthcare team. Experience in Rheumatology is preferred. You will work closely with physicians and other healthcare professionals to diagnose and treat patients, as well as educate them on their conditions and treatment options.

```Duties:```
- Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests
- Develop and implement patient care plans in collaboration with the healthcare team
- Provide ongoing monitoring and management of patients' conditions
- Administer medications and treatments as prescribed
- Educate patients and their families on preventive healthcare measures, disease management, and treatment options
- Utilize evidence-based practice guidelines and standards to deliver quality care
- Document all patient interactions, assessments, interventions, and outcomes accurately and timely

```Skills:```
- Proficient in patient assessment, diagnosis, treatment and medical documentation
- Experience in rheumatology is preferred
- Excellent communication skills to effectively educate patients on their conditions and treatment plans
- Ability to work collaboratively with a multidisciplinary healthcare team
