Registered Nurse:

Provide comprehensive assessments, develop a patient-specific plan of care, coordinate the individual plans and evaluate service. This position involves visiting the client in their “home” environment. Work with an interdisciplinary team, including but not limited to: Physical Therapist, Occupational Therapist, Speech Therapist, Social Workers, Registered Nurses, Licensed Practical Nurses, Home Health Aides, Medical Director, and administrative staff.

Position Qualifications:

Current Registered Nurse state license. Demonstrate clinical competency and show evidence of ongoing professional educational training.

Essential Duties and Responsibilities

• Create and implement care plans – Develop a care plan specific to the client’s needs and desired outcomes

• Assess each client’s status individually. Complete a thorough assessment of the client's medical health, the safety of the home environment, ongoing care management, etc.

• Act as a liaison, client advocate and coordinate among those involved with client’s situation, including health care providers and different multidisciplinary teams, including client's physician

• Provide ongoing education to clients and family members on changing care plans

• Obtain and implement physician orders

Job Types: Part-time, PRN

Pay: $45.00 - $100.00 per hour
