Remarkable Hospice Companies is Hiring a Magnolia Hospice Austin - Patient Care Coordinator Near Austin, TX

General Purpose:

Responsible for managing accurate, timely completion and submission of a variety of data entry including patient information, daily reporting, and the paper flow of clerical functions in the office. 

Essential Functions:

  • Schedules and appropriately documents patient schedules in a timely manner. 
  • Participates in coordinating care with management, patient, and interdisciplinary group.
  • Provides effective communication to patients, staff members, other health care professionals and referral sources.
  • Responds to Agency and patient needs in a professional and creative manner.
  • Demonstrates commitment, professional growth and competency.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
  • Receives and appropriately documents new referrals in a timely manner.
  • Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
  • Provides effective communication to patients, staff members, other health care professionals and referral sources.
  • Responds to Agency and patient needs in a professional and creative manner.
  • Demonstrates commitment, professional growth and competency.
  • Participates in Agency-sponsored in-service training.
  • Participates in the Agency's QAPI program.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.

Minimum Education & Experience Requirements:

  • High school graduate or equivalent (college preferred)
  • Two years general office management and human resource experience preferred
  • If an LVN/LPN, nursing skills as defined as generally accepted standards of practice. 

Knowledge, Skills & Abilities Required:

  • Knowledge of office machines preferred
  • Computer skills required, excellent interpersonal and organization skill
  • Good interpersonal skills
  • Light typing preferred
  • Reliable transportation
  • If an LVN/LPN, one year experience preferred.
