Field Supervisor is responsible for planning, coordinating, and documenting skilled supervisory visits NON- Oasis for the Delaware County and surrounding county areas of pediatric clients following PA Home Health regulations PA Chap 601. Field Supervisors assure compliance with local, state, and federal laws, Medicare regulations, and established company policies and procedures. The field supervisor is responsible for the direct supervision of the LPN and RN in Field. ESSENTIAL DUTIES AND RESPONSIBILITIES 1. RN must be able to complete SOC which include admission demographics and develops the initial care plan. POC / 485 and NON Oasis ( EMR ALORA) 2. Confirms client’s preferred schedule for care and follows up with clients to ensure that quality care is received. 3. Complete client Recertifications and notifies DON of Quality Assurance issues and of any changes in the client's condition that may necessitate changes to the established care plan. 4. Train Newly hired staff as needed and orient staff to case. 5. Assists in closing client care gaps and conducts unannounced home visits as necessary. 6. Communicates client problems, concerns, changes, and complaints to appropriate staff and prepares reports of incidents that could result in legal action. 7. Maintains and develops positive relationships with existing and prospective clients, demonstrating excellent customer service and setting an example for other staff. 8. Maintains daily schedule in HHAX and accurately completes visits in a timely manner. 9. Participates in staff meetings, agency sponsored in-services, and team meetings, as directed. 10. Performs other duties and assists with special projects as requested. REQUIRED SKILLS AND ABILITIES

EDUCATION AND EXPERIENCE
Graduate from an accredited School in Nursing- required
Active RN License in good standing in the State of Pennsylvania- required
At least one (1) year of previous experience in health home health as supervisor.
Proficient with Microsoft Office Suite or related software.
Must have the use of an automobile in good working condition, a valid driver's license, and automobile insurance compliant with company policies.
PHYSICAL REQUIREMENTS
Ability to move, transport, or position up to 50 pounds
Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.
Ability to communicate effectively and clearly with others to exchange information.
TRAVEL REQUIREMENTS
Regular travel on a daily or weekly basis is required even in inclement weather.
