Our Intake team performs intake procedures including (but not limited to) identifying & evaluating patients referred for services, assisting with the authorization & caseload building process, and communicating with occupational, physical, and speech therapists regarding their Home Health clients.
Duties & Responsibilities:
· Be primary point of contact with patient family regarding staffing schedules.
· Maintain compliance with federal and state regulations as it pertains to the agency.
· Work closely with office staff to promote positive/effective communication to ensure appropriate client care.
· Set and model quality customer service standards.
· Proactively follow up with families while managing and maintaining staffing schedules.
· Assume responsibility in coordinating care to assigned patients, working to support a goal directed care plan from admission to discharge.
· Assist with answering in-coming telephone calls from Caregivers and Clients.
· Work in collaboration with Client Service Manger to maintain client relations and service failures
· Demonstrate ability to prioritize multiple tasks to meet deadlines.
· Demonstrate ability to interact in a collaborative manner with other departments and teams.
· Demonstrate an ability to identify and solve problems; uses initiative and good judgment to reach quality decisions.
· Demonstrate a thorough knowledge and adherence to company policies and procedures.
· Maintain an excellent rapport with patients and staff, and effectively promotes harmonious interpersonal relationships.
· Assist sales and marketing with paperwork as needed.
