Unit Director - Community Support Program (CSP) - $24.04/Hr

Closing Date: 04/29/2024

FT 40 Hr/wk Provides positive leadership to direct care employees whom are working within the community to provide case management to program participants. The unit director is responsible to ensure the program operates in compliance with contracted outcomes and accreditation standards. Maintaining quality records, running reports, reviewing and approving documents while working within our ERMS system required. Hours: Mon - Fri, 8:00 am - 4:00 pm

Duties and Responsibilities:

  • Ensures program activities, goals and communication are in compliance with agency standards
  • Recruits, leads, and develops designated coordinator(s), and direct care staff
  • Provides direction, assistance, and approval to coordinators and direct care staff for major crisis intervention in consultation with clinicians
  • Conducts program admission
  • Ensures the initial IRP is completed within 30 days
  • Ensures assessments (IRP/Crisis plan) are completed and provides program approval for admissions, discharges in collaboration with clinicians, program coordinator or service coordinators
  • Designates staff assignments for program participants in conjunction with program coordinator
  • Ensures data is entered into HMIS by either Director or Program Coordinator by due dates as per program contract requirements
  • Attends Agency Intake meeting or designates program coordinator that can make a final decision. The service director also may attend
  • Attends team meeting in collaboration with clinicians
  • Provides input for budget and manages Program within budget guidelines
  • Provides required reporting for funding purposes
  • Assures coordination of services among designated programs, other Reliance Health Programs, and community programs
  • Implements policies, procedures, and program evaluation for designated programs
  • Identifies programmatic and resource needs to support program expansion
  • Monitors Quality Assurance/Quality Improvement for service area
  • Ensures employee evaluations are up to date and evident in personnel files on direct reporting staff and per diems used in the program
  • Meets deadlines in completing and submitting required/requested documentation
  • Provides scheduling for adequate coverage of the program
  • Reviews notes weekly, make corrections and teach employees content, accuracy and appropriate terminology
  • Provides regular supervision that is documented and evidenced in Personnel files by submitting to HR at the time of supervision and by eval deadline of 3/1 or 9/1. Supervision is acknowledged by the signature of employee using the Agency form that includes but is not limited to: providing honest and fair feedback about performance, discussing employee goals & growth, appreciation and program communication
  • Approves employee timesheets accurately prior to 9am on Monday payroll weeks and when appropriate, approve and fax per diem timesheets prior to 9am Monday payroll weeks
  • Provides AIC/CIC/SIC duties as assigned
  • Engages in continuous concerted efforts to access, obtain, respond to pertinent agency information/communications and disseminate as appropriate
  • Provides working conditions and education that maximizes safety for coordinator, direct care staff and program participants
  • Ensures program safety measures are met on a regular basis
  • Other responsibilities of Reliance Health staff, such as participation in Agency events, adhering to Agency Policies & procedures, training requirements etc.
  • Performs duties while demonstrating Agency Core Competencies and values

Education and Experience:

  • Bachelor’s degree in Human Services or equivalent work experience in the provision of mental healthservices
  • 5 or more years previous work experience required
  • Experience supervising others preferred
  • Valid CT Driver’s License, vehicle registration and insurance required
  • Good driving record
