Chart Abstraction:

• Carefully abstract and input patient information from handwritten physician notes into the Electronic Health Record (EHR) system.
• Ensure the accuracy and timeliness of all entered data, upholding high standards of precision.

Medical Record Analysis:

• Examine medical records for thoroughness and correctness.
• Address any inconsistencies or gaps in patient charts by liaising with healthcare professionals.

Compliance and Privacy:

• Strictly adhere to patient privacy and data protection standards in line with HIPAA regulations, particularly when working remotely.
• Remain current with medical documentation norms and updates to the EHR system.
• Interprofessional Collaboration:
• Work closely with medical personnel to clarify any ambiguous or uncertain information found in the notes.
• Offer assistance to healthcare staff on documentation procedures and EHR system use.

Qualifications:

• Minimum of a high school diploma or its equivalent; an associate degree in health information management or a similar field is preferred.
• Demonstrated background in chart abstraction, medical transcription, or a related healthcare role.
• Knowledge of medical terminologies and abbreviations.
• Proficiency in EHR systems and Microsoft Office applications.
• Exceptional typing abilities and a keen eye for detail.
• Strong communication and organizational skills.
• The capacity to function autonomously and collaboratively, especially when working remotely.
• Dedication to upholding ethical standards in healthcare and ensuring patient confidentiality.

Physical Requirements:

• Must possess an appropriate home office setup that is conducive to professional tasks.
• Capability to remain seated for prolonged periods.
