Medication Specialist Hours of Work: Full-time. Non-Exempt. Minimum Qualification: High School Diploma plus one year in health related field and human services, proficient with MS Word and Excel. Source of Supervision: Program Manager Essential Functions: The Medication Specialist will provide coordination of medication training, support to tenants, and oversight of all medication practices including the following: 1. Responsible for a designated case load (Approximately 25 tenants). 2. Supervise and monitor medication management procedures. 3. Order medications/refills. 4. Conduct weekly medication counts. 5. Consult with physicians/treatment providers, pharmacy regarding medication concerns. 6. Obtain prescription/doctor’s orders. 7. Review provider appointment forms and hospital discharge documentation for any medication changes. 8. Log in medications upon their delivery. 9. Update medication sheets with any new medications or medication adjustments throughout the month. 10. Provide tenant education regarding medication names, dosages, and side effects to tenants. 11. Document medication supervision outcomes and/or significant events. 12. Change medication books at the end of the month. 13. Report to supervisor problems including behavioral concerns. 14. Accompany tenants to appointments and/or activities. 15. Provide coordination and assistance in crisis intervention and stabilization. 16. Schedule appointments. 17. Communicate medication changes to all staff (written and verbal). 18. Work collaboratively with all internal and external entities, and participant collaterals includes family, significant others, and medical & behavioral health treatment providers. 19. Responsible for the accurate and timely completion of case record documentation in the Agency electronic health record. 20. Attend staff meetings as scheduled, which includes discussing topics that impact the program, Agency, and tenants. 21. Participate in individual supervisory meetings 22. Complies with HIPPA requirements to ensure confidentiality and the protection of all client civil liberties, rights, and property. 23. Comply with all workplace policies & procedures. Other Duties: 1. Attend/participate in any/all required trainings related to the responsibilities of the position. 2. Performs duties assigned by the immediate supervisor or other management staff, as required.
