Case Coordinator DTAP
Rehabilitation Support Services, Inc. is hiring a Case Coordinator for its Dutchess Treatment Apartment Program (DTAP). Hours and Benefits:
· 40 hour work week. 8:30am-5pm Tuesday- Saturday and Sunday-Thursday .

Requirements for the Case Coordinator:
· A high school diploma or equivalent
· Valid NYS Driver’s License required for most positions.
· Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check also required.
Responsibilities for the Case Coordinator include:
· Responsible for a designated case load; including the accurate and timely completion of case record documentation in the Agency electronic health record.
· Establish and maintain good rapport and therapeutic relationship with residents.
· Aid and encourage clients to meet the established goals of their service plans.
· Teach, monitor and evaluate client’s skills of daily living.
- Encourage client decision-making.
- Report client problems and behavioral changes to supervisor and clinical providers.
- Provide advocacy services, as needed, regarding applications for benefits, medical services and clinical services.
- Attend treatment and medication reviews, with clients, as needed.
- Monitor client medical and mental health and coordinate ongoing medical care and services.
- Provide crisis intervention services, as necessary.
- Complies with HIPPA requirements to ensure confidentiality and the protection of all client civil liberties, rights, and property.
- Attend scheduled staff meetings.
-Participate in supervisory meetings.
-Supervise/monitor self-medication of clients and conduct supervised medication rotation weekly (example: medication run).
- Ensure all health and safety procedures are maintained.
-Ensure clients are transported to needed services in timely manner.
-Conduct site visits, in accordance with program requirements, to ensure housekeeping standards are met and to teach/monitor clients skills of daily living.
-Work collaboratively with all internal and external entities, and participant collaterals including family, significant others, and medical and behavioral health treatment providers.
-Participate in client-centered meetings, as scheduled.
-Assist with movieng clients in/out of apartments, when necessary.
-Involve clients in community activities.
-Facilitate MICA education, if applicable.
-Complete random urinary drug screenings, if applicable.
-Conduct relapse prevention support groups, if applicable.
-Comply with all workplace policies and procedures.
-On-call responsibilities, as necessary.
