Job Overview:
Leadership role requiring excellent communication skills and is responsible for the supervision and coordination of all interdisciplinary hospice services to patients and families in the community. This position has the primary authority and responsibility for maintaining the quality of clinical services, across disciplines in the home hospice setting.
Responsibilities:
- Coordination & management of all nursing and interdisciplinary services rendered to patients and families.
-Supervision of clinical supervisors and nursing personnel in the planning and delivery of services to patients & families.
- Development of agency policies and procedures in collaboration with Quality Administrator and the Medical Professional Advisory Committee.
-Ensures effective admission and discharge planning and coordination of hospice care services with contracted facilities, such as skilled nursing and assisted living facilities. -Assesses community health needs and requests for programs and services.
-Maintains compliance with licensure regulations and applicable clinical standards.
-Development and implementation of General Inpatient (GIP) partnership(s) with outside agencies.
-Oversees the electronic medical record system.
Skills:
- Patient monitoring and assessment skills
- Supervisory skills for managing a team of caregivers
- Knowledge of medical terminology related to hospice care
- Ability to manage shifts effectively for optimal patient care coverage
- Understanding of home care services and assisted living facilities
- Nurse management skills for coordinating patient care across multiple settings
- Experience in developing individualized care plans based on patient needs
- Budgeting skills for managing resources effectively
Qualifications:
- RN with BSN required/MSN desired and active CT License
- Minimum of 5 years supervisory or administrative experience in a healthcare setting, preferably providing hospice care
- Hospice/Palliative Care Certification or eligibility
- Knowledge of applicable State and Federal regulations
- Strong interpersonal, communications, and computer skills required.
