Population Health Support Specialist

ReGenesis Health Care is seeking a Full Time Population Health Support Specialist to join our well-established practice serving Spartanburg, Cherokee, and Union counties in SC. The Population Health Support Specialist will work in partnership with the Value Based Care Manager, Quality Improvement Manager, and other population health staff to provide front line support for value-based care efforts across the organization. We are a Federally Qualified Health Center (FQHC) offering an array of services to meet the needs of the communities we serve. We operate nine locations across our three-county service area offering, Primary & Dental Care, Behavioral Health services, Pharmacy, Patient Transportation, Mobile Medical, and Outreach Services. 

This is a non-exempt position that offers a competitive hourly pay rate up to $17.00 per hour. Quarterly bonus incentives are paid to all staff that meet defined eligibility criteria. Your eligibility in our comprehensive benefits program will begin the first day of the month following your official start date.

Population Health Support Specialist Duties/Responsibilities:

  • Provide front line support to address and ensure HEDIS measures are met, including but not limited to face-to-face patient visits, outreach phone calls, data collection and analysis.
  • Measure and monitor hospital utilization.
  • Support chronic disease management programs.
  • Assess patients for health-related social needs.
  • Coordinate patient care by providing referrals for internal and external support.
  • Create and maintain reports for tracking metrics regarding success of value-based care efforts.

Required Skills/Abilities:

  • Customer service-oriented mindset.
  • Experience with quality improvement and assurance programs.
  • Excellent verbal and written communication skills.
  • Excellent time management skills with a proven ability to meet deadlines.
  • Proficient with or the ability to quickly learn essential software and electronic platforms for data entry, tracking, and documentation.
  • Proficient with Microsoft Office Suite or related software.
  • Excellent interpersonal skills and willingness to work in a team environment.
  • Ability to lead by example.

Education and Experience:

  • 1-2 years of related healthcare experience.
  • High school diploma.
