MDS Coordinator

We are currently seeking an experienced MDS Coordinator, who can be a Registered Nurse (RN) or Licensed Practical Nurse (LPN), to join our team. The MDS Coordinator will be responsible for ensuring the accurate and timely completion of the Minimum Data Set assessments and care plans, which are crucial for our resident's care and the facility's compliance with state and federal regulations.
**Key Responsibilities:**
- Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment.
- Monitor and guide the completion of MDS assessments to ensure accuracy and timeliness.
- Collaborate with the interdisciplinary team to develop and review personalized care plans for residents.
- Ensure compliance with federal and state regulations in all aspects of MDS coordination.
- Train and support nursing staff in MDS process best practices.
- Participate in quality assurance and improvement processes related to resident care and MDS completion.
**Qualifications:**
- Valid RN or LPN MA license.
- Proven experience as an MDS Coordinator in a long-term care facility.
- In-depth knowledge of MDS 3.0 and care plan development.
- Strong organizational, communication, and leadership skills.
- Detail-oriented with a commitment to accuracy.
- Ability to work collaboratively in an interdisciplinary team environment.
