Title: Registered Nurse Case Manager - Hospice
Status: Full Time
Shift: 8:00 am - 6:00 pm every Monday, Wednesday, and Friday with the flexibility to pre-schedule an off-day on Tuesday or Thursday!

What will I do as an employee with Redstone?
As a RN Case Manager - Hospice, supporting the Redstone mission will include the following responsibilities:
- Provides nursing care to clients in a variety of settings. Responsible for the implementation of services to meet client needs. Follows current nursing professional standards of practice and agency approved protocols for home health, hospice and home care.
- Provides ongoing assessment of clients, assuring an accurate evaluation of client needs. Identifies when clients need or qualify for additional services and makes referral to appropriate manager or other community resource. Assesses client and caregiver needs, provides client and caregiver education.
- Maintains current and accurate client records through use of laptop computers and paper/chart documentation in accordance with corporate policies, federal, state, and local guidelines.
- Collaborates with Interdisciplinary team members and attends required meetings. Performs supervisory visits of Personal Care Aides, Home Health Aides, Homemaker/Companion, and/or Licensed Practical Nurses assuring the delivery of quality health care services per program policies.
- Completes home visits and on-call duties after normal business hours, on weekends, and holidays as needed or assigned. Conducts wellness visits as assigned.
What do I need to qualify for this role with Redstone?
- Current Registered Nurse license in Pennsylvania.
- A valid Pennsylvania Driver's license in good standing.
