Reporting to the Director of Case Management, this role of this position is to assist in creating a seamless flow through the health care system for the orthopedic surgical patient. The Orthopedic Patient Navigator provides one-on-one guidance and coordination of services and helps to eliminate barriers to the patient going through the surgical procedure. The Orthopedic Patient Navigator will follow patients through the care continuum, from entry into the system through discharge and post procedure follow up. The Orthopedic Patient Navigator will serve as a patient advocate, guiding patients through and around barriers in the system.
QUALIFICATIONS:
1. Current California RN license.
2. Current CPR required (American Heart Association only)
3. Bachelor’s degree in nursing preferred.
4. Three years in acute Med/Surg nursing preferred.
5. Orthopedic nursing experience preferred.
6. Previous experience in case management in a hospital setting preferred.
