ReDiscover is seeking an Enhanced Care Pathways (ECP) Coordinator to add to our Access/Intake/Crisis Department!

ReDiscover has been known for over 50 years as a non-profit community mental health center in southeastern Jackson County, Missouri with a mission of delivering efficient and cost-effective treatment for the people we serve. Our diverse team of over 700 employees is “Passionate About People” as we support our community, specifically those whose lives have been affected by a mental illness or substance use.

WHAT WE DO

Access, Crisis & Outreach helps community members connect to mental health services in time of need or in times of crisis. This department aims to resolve and remove any barriers for individuals as they connect to the services they desire. The ECP team works with all ReDiscover programs to create a safety net and provide increased support for clients who are at risk for suicide.

At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:

• Providing assertive, intensive engagement to people in the community who have been identified at risk for death by suicide.
• Facilitating and engaging clients on a short term basis to find suitable programmatic solutions that best align with the client’s needs.
• Engaging with ReDiscover employees and community partners to ensure there is an enhanced safety net for the client’s being served.
• Collect and maintain information about clients and document interactions within ReDiscover’s designated electronic medical record.
• Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

We are focused on your development and will spend time helping you continue to grow as an ECP Coordinator. However, here are some things that you will need before starting with ReDiscover:

• Bachelor’s degree in a social science field, 4 years of equivalent experience in lieu of degree, or a combination of experience and education.
• A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work.
• Working knowledge of mental illness and substance use disorders.
• Excellent customer service skills with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as G-Suite.
