Monday - Friday: 8:30am-5:00pm Contract to Hire - $22/hr.
Description:
The Health Navigator (HN) is primarily responsible for delivering quality services to eligible clients to support the achievement of safety, well-being, and permanency objective. These services are intended to help prevent the need for more restrictive and/or higher intensity services for eligible clients. The HN would utilize a systemic "Circle of Support" approach in providing services to clients. The HN would assist clients to develop social and independent living skills and help them respond to situations that could trigger a crisis. The HN would assist clients with coping skills, hygiene, self-care, and ways to appropriately respond to health care issues. The HN is expected to use an individualized service delivery approach, ensuring that services are designed to address the unique needs and strengths of clients.
Required Qualifications and Experience at This Level:
• Bachelor's Degree in a related field.
• Must be culturally competent.
• Refugee resettlement program experience a plus.
• Solid organizational and follow-up skills.
• Computer literacy - learn and adapt to different IT platforms.
• Must be fluent in English.
• Being bilingual in at least one other language such as, Farsi, Dari, Pashto, French, Swahili, Spanish or any other foreign languages, is required.
• High degree of independent judgment, flexibility, and problem-solving skills.
• Must have a valid driver's license, automobile liability insurance, and knowledge of the city.
• Some evenings or weekends may be required.
