Recruit Secure is collaborating with a beautiful and highly-rated Skilled Nursing Facility (SNF) located in Arverne, NY area that is seeking a dynamic and energetic individual to take on the full-time role of a Director of Social Services. The SNF is a long-term nursing and rehabilitation facility with about 100 beds and is dedicated to providing the highest levels of care and commitment to its residents, their families, and their employees. The salary range is dependent on experience with plenty of room for growth. A full benefits package is included.

Responsibilities:

• Assists in Hiring, and Managing and is a resource to the Social Services team.
• Sets goals for the department, and communicates clearly, and meets regularly with the Social services team
• Assures that policies and procedures are adhered to for the SNF; makes recommendations, updates, and revisions to policies when and if needed.
• Develop the Social Services plan of care for residents, identifying goals and approaches and participating in inter-department care planning meetings.
• Participates in Quality processes of the SNF
• Counseling residents and family members; facilitate family meetings
• Complete Admissions, Psychosocial Assessments, and Advanced Directives and assures placement in medical records.
• Oversees referrals for health services and facilitates the discharge planning process for the department
• Completing all required documentation for medical records; producing monthly departmental reports for the SNF Administrator.

Requirements:

• Master’s degree in Social Work, LMSW preferred but not mandatory
• Must have at least 4- 5 years experience working as a Licensed Social Worker within a Nursing or Rehabilitation Facility.
• At least 1 year’s experience in a Senior or Director role
• Should have a good working knowledge of available social services and other community resources
• Must have excellent communication
