Job Description:

Nursing Home Social Worker

Plaza West Healthcare and Rehab Center is seeking a Full-time Social Services Coordinator to join the social services team at our skilled nursing facility in Topeka, KS. This position is an integral part of the interdisciplinary team. This position offers the opportunity to serve the elderly and their families in a fast paced environment.

Duties:

  • Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs during their stay at Plaza West.
  • Developing and Maintaining individualized care plans for rehab residents.
  • Participating in the planning of the resident's community re-entry or transfer to another facility.
  • Participating in interdisciplinary team to ensure resident’s discharge goals are met.
  • Assist residents/families with different payor sources such as: KanCare, Private, Medicare, etc
  • Documentation in the resident’s clinical record
  • Refer and assist residents and families to secure community-based aid and services
  • Communicate with families as needed to involve them in the resident’s care. 

REQUIREMENTS:

  • Current Kansas License (preferred)
  • Social Service Designee (SSD)
  • EXPERIENCE WITH MDS 3.0 (preferred)
  • Long term care/ sub-acute experience (preferred)
