Job Description: Home Health LVN/LPN

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a home health nurse. This position is contract-based, and you will be compensated per visit as a 1099 independent contractor. As a home health LVN, you will provide quality nursing care to patients in the comfort of their own homes.

Key Responsibilities:

• Conduct comprehensive assessments of patients' health status.
• Develop and implement individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor and document patients' progress and responses to treatments.
• Educate patients and their families on disease management, home care, and healthy lifestyle choices.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and effective care.
• Maintain accurate and timely documentation in accordance with company policies and regulatory requirements.

Qualifications:

• Current and valid LVN license in Texas.
• Minimum of 1 year of nursing experience, preferably in home health or a related field.
• Strong clinical skills and the ability to provide high-quality patient care.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and willingness to travel to patients' homes within the service area.
• Basic computer skills for documentation and communication purposes.
• Kinnser EMR experience preferred.
