We are looking for a mission driven Diabetes Care and Education Specialist to join our team! The diabetes care and education specialist (DCES) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes care. The DCES is an integral part of the inter-professional team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan. The DCES delivers case management services to patients to ensure cost effective use of resources and positive patient outcomes. The DCES supports and advocates for people affected by diabetes to optimize quality care.

This is a full-time position located on our main campus (30th & L). The schedule will be 40 hours per week; generally, Monday-Friday, 8am to 5pm.

To ensure the safety of our patients, employees, and communities, OneWorld Community Health Centers is recommending all employees to be fully vaccinated against COVID-19.

QUALIFICATIONS:

  1. Licensed by the State of Nebraska or multistate license as a Registered Nurse required, OR Registered Dietitian certification through the Commission of Dietetic Registration required.
  2. Diabetes self-management education and support experience preferred. If not already DCES will sit for the certification examination within 24 months of hire date.
  3. Minimum of one year of experience working as a Registered Nurse or Registered Dietitian required.
  4. Proficiency in both English and Spanish required.
  5. Expertise in diabetes, nutrition, physical activity, motivational interviewing, weight reduction, cholesterol management, stress management, and/or hypertension management preferred.
  6. Proficient computer skills in Microsoft Office products (Word, Excel, PowerPoint, etc.) required; prior experience with an Electronic Health Record preferred.
  7. BLS certification required for Registered Nurses.
  8. Demonstrated ability to manage multiple tasks.
  9. Demonstrated ability to work with minimal supervision independently and as a team member.
