Job Description I-RISE Case Manager - Interfaith-RISE - Reformed Church of Highland Park Affordable Housing Corporation

Position Description

This position will be located in both the Highland Park and Neptune (Monmouth County) satellite office.

The Case Manager provides case management to refugee families, inclusive of pre-arrival planning through the first 90 days of core services, and services to longer-term resettlement clients. The Case Manager will be responsible for collaborating with service directors to ensure clients work toward achieving their goals and attaining self-sufficiency. Case managers will be assigned to Reception and Placement, Refugee Support Services, and/or Refugee Cash Assistance. 

This position is required bilingual English/Creole.

Reporting to the I-RISE Director of Case Management (R&P, RSS) or the Executive Director of I-RISE (RCA), this position is 40 hours/week at $47,500/year plus health insurance and benefits.

Responsibilities Include

Services for First 90 Days (Reception and Placement):

  • Provide high-quality case management services for all arriving refugees. Coordinate and deliver all necessary services within the 90-day service time period post-arrival
  • Work with the Volunteer Coordinator to connect volunteer community members with clients to continue as point-people for families to ensure a continuum of care post-90-days
  • Help refugee youth and their families connect to the larger community, cultivate cultural awareness for both the refugees and the communities in which they resettle, promote civic engagement, and enhance language skills.
  • Provide families with culturally and linguistically appropriate materials on programs or activities, translation services, and interpreter services
  • Provide family connection to services and benefits
  • Facilitate family participation/enrollment in programming that supports integration into the community, such as English as a Second Language (ESL) sessions, mentoring, and group activities
  • Collect and input client and case management data as needed using Apricot and other systems
  • Assist case managers and other I-RISE team members as assigned by the I-RISE Director
  • Other responsibilities as assigned

For Longer-Term Clients (Refugee Support Services):

  • Provide comprehensive case management services to clients with barriers to self-sufficiency
  • Upon receipt of referral from internal and external sources, schedule an intake appointment, and arrange interpretation and transportation for the client, as necessary
  • Conduct intake, determine eligibility, complete assessment, and orientation, and assist client in developing their self-sufficiency plan
  • Refer clients to appropriate internal and external services and programs based on their needs
  • Provide linkages to agency, community and government services and monitoring referral completion including state benefits such as SNAP, Medicaid, WIC, SSI,
  • Follow up with clients monthly to track their progress and update their self-sufficiency plan
  • Advocate for clients to ensure they receive culturally and linguistically appropriate services
  • Complete documentation in case management database systems in a timely manner
  • Follow all RCHP-AHC/Interfaith-RISE policies
  • Other responsibilities as assigned

Refugee Cash Assistance

  • Processing application approvals and program enrollments with NJOR
  • Coordinating with RCA Program Director to assist Case Managers to enroll clients in the program, collect needed client information and data, and request RCA checks from RCHP-AHC finance department.
  • Coordinating with RCA Program Director and Interfaith-RISE Director of Case Management to manage Case Managers implementing client Refugee Resettlement Services (RSS) to clients
  • Regular and timely case noting and updating client data in internal and externally required database management systems, including Apricot and Client Track
  • Supporting the RCA Program Director to oversee the RCA database for the storage of client files, program enrollment forms, monthly check-ins, client payment processing, and reporting, etc.
  • Coordinating with RCA Program Director to collect and prepare data for regular financial and programmatic reports required by funders
  • Report any client issues to the Case Managers for appropriate follow-up
  • Other responsibilities as assigned

Qualifications

Bachelors in social work - Master’s preferred - or equivalent work experience. Excellent written and oral communication skills. Experience working with diverse populations, including non-English speaking clients, and individuals from various faith, cultural, and social backgrounds. Ability to multi-task and creatively problem solve, and ability to work in a fast-paced and vibrant environment required. Bilingual English/Creole required.
