Case Managers provide goal-directed supports and solution-focused interventions to individuals diagnosed with a mentally illness and/ or a substance use disorder. Case managers work with clients, family, support networks and other collaterals and providers to support on-going monitoring, care, and training in self-care, social integration, skill-building, and health maintenance. Case managers help clients to identify and minimize problems which interfere with the client’s interpersonal relationships, financial management, healthy recovery and school or job performance.

SUD Case Manager/Therapeutic Behavioral Specialist Job Duties:

· Provide support to clients in their path to sobriety and recovery. Identifying short-term and long-term goals for recovery and outlining strategies to achieve them.

· Assist with level of care recommendation placement. Connecting clients with appropriate resources such as detoxification facilities, residential treatment programs, outpatient services, mental health professionals, and support groups.

· Providing education to clients and their families about substance use disorders, treatment options, and relapse prevention strategies.

· Teaching practical skills for coping with triggers, managing cravings, and building a healthy lifestyle.

· Assisting client with legal matters including working with probation, organizing court appearances, court fines etc.

· Service/treatment planning, care coordination, and/or referral and linkage for healthcare, behavioral healthcare, or non-healthcare services

· Acting as an advocate for clients to ensure their needs are addressed within the treatment system. Providing ongoing support and encouragement to clients as they navigate the challenges of recovery.

· Problem solving and navigation of services with clients and their families.

· Identification of strategies or treatment options

· Restoration of social skills and daily functioning

· Assisting clients in managing crises or relapses by providing immediate support and connecting them with appropriate services.

· Establishment of trust and rapport, active listening, empathy and goal-setting

· Coordinating services with other providers

· Participating with other clinical staff as a member of our interdisciplinary treatment team

SUD Case Manager/Therapeutic Behavioral Specialist Educational Requirements:

· Experience with mental health and substance use disorders

· Must have a bachelors or a masters degree in social work, psychology, nursing, or in related human services; or a high school diploma with a minimum of three years of relevant experience.

SUD Case Manager/Therapeutic Behavioral Specialist Position Requirements:

· Valid driver's license

· Must be able to pass background check
