Rappahannock Area Community Services Board is Hiring a Nurse, LPN - ICF - Lucas Near Fredericksburg, VA

OVERVIEW

Provides general nursing care and medication management and supervises, supports and instructs intellectually-disabled and developmentally-disabled adults having more complex medical or behavioral needs in a group home setting. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, Board of Pharmacy and Board of Nursing regulations, state and federal licensure and accreditation regulations and guidelines.

DUTIES AND RESPONSIBILITIES

  • Provides general nursing care.
  • Assists with scheduling and supports psychiatric services.
  • Provides routine examinations such as measuring blood pressure, pulse, temperature, weight and blood glucose readings
  • Dispenses medications and administers injections and maintains required records.
  • Records data, prepares and maintains detailed case records, activity reports and other records.
  • Provides counseling and educational information regarding medication use, diet, health maintenance, etc. to individual and/or individual families and care givers.
  • Prepares medical correspondence and reports.
  • Maintains adequate first aid supplies and materials.
  • Responds to medical emergencies and other potential crisis situations.
  • Orders, inventories and secures pharmaceuticals.
  • Attends required team meetings, staff meetings, conferences, and in service training.
  • Provides resident and resident's family medication management and behavioral support training.
  • Observes, documents and reports reactions to medication/treatments and significant behavioral incidents.
  • Assists with admission process and the development and implementation of treatment plans.
  • Monitors drug count and maintains records.
  • Assists individuals with personal hygiene and grooming.
  • Monitors, instructs and assists residents with eating, daily living skills, mobility, communication skills and community interactions and activities.
  • Records resident's daily progress and communicates resident's behavior to relieving and/or management staff.
  • Reports resident behavior to other service providers, family members, legal guardian and other authorized parties.
  • Facilitates, networking and support for residents engaged in volunteer, social and leisure activities.
  • Provides overnight support and supervision to residents and monitors resident activities.
  • Participates in housekeeping and meal preparation activities.
  • Assists in behavioral interventions.
  • Transports or escorts individuals to various activities and to and from the program.

KNOWLEDGE, SKILLS, AND ABILITIES

Considerable knowledge of the principles, standards, practices and techniques related to general nursing; considerable knowledge of Board of Pharmacy and Board of Nursing regulations relating to prescription drug storage, administration and documentation; considerable knowledge of the principles, practices and techniques of instruction, support, and behavior management for persons with intellectual disabilities; some knowledge of common housekeeping practices; some knowledge of communicable and other infectious diseases, ability to establish and maintain rapport with the individual population; ability to establish and maintain effective working relationships with agency clinicians and staff of other private and public organizations; ability to function calmly in stressful situations; ability to operate a vehicle; ability to observe, assess and record individual behaviors and functioning.

TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCE

Associate's Degree or completion of technical training program in Nursing and some professional experience in nursing and/or behavioral health serving the intellectually-disabled population.

LICENSURE AND CERTIFICATIONS

State Certified Licensed Practical Nurse. Valid Virginia Driver's License; current First Aid and CPR certification.
