Job Summary: Implementation, coordination, and supervision of applicable aspects of a comprehensive Health, Wellness andClinical Program which addresses the needs of all Assisted Living Residents inaccordance with Victory Housing, Inc. Policies and Procedures, COMAR 10.07.14and Board of Nursing Regulations and Standards. Oversight includes Providewellness assessments, care planning, care oversight and quality assurance forall residents. Maintain all charts toinclude, progress notes, identification of nursing interventions, treatments,and assessments in coordination with and supervision by the Delegating RN. Implement a Wellness Program to include weekly scheduled resident Health and Wellness Check-ups, to ensure a proactive and preventativewellness centered and holistic approach. Oversightand coordination of all third party vendors to include but not limited toPodiatrists, Dentists, Lab, Pharmacy, Home Health, Hospice etc. Fulfill the role of Wellness Leader in the community as an essential manager, resident advocate, and casemanager. Reports to : Delegating RN, Direct on clinical Executive Director of Assisted Living Community, Direct on day today; Indirect on clinical Supervises : Supervisionand implementation of health, wellness and clinical programs excluding the responsibilitiesof the Delegating Nurse function. Areas of Responsibilities :
Ensure that the attainment of Victory Housing's Mission Statement takesprecedence over all decisions and actions
Assume the responsibilities of the Case Manager
Maintain overall quality of the resident care program in accordance with all local, state, and federal regulations and the highest standards of nursingpractice
Partners with the Assistant Director on all aspects of resident servicedelivery, needs, individualized service plan development and care tracker
Under the delegation and partnership of the RN, assist in monthlymedication crossover, maintain the medication carts and assist with theimplementation of other pharmacy policies and procedures allowable byeducation, licensure and/or certification by the Maryland Board of Nursing
Utilize Eldermark per Victory Housing standards and policies and procedures
Work closely with all staff on health and wellness issues. Proactively intervene whenever a statuschange is assessed
Implement a comprehensive Health and Wellness Program designed toproactively address physical, psychosocial and rehabilitative needs of the residents. Include quality assurance and outcome measureswhich monitor trends and anticipate potential problem areas
Schedule weekly Check-ups with each resident to monitor all aspects of the resident's conditionand intervene as indicated. Review Medications, Fall Risk, Vital Signs, Weights, Nutritional Status, Skin Assessments and as well asoverall Clinical, Psychological and Social Status Changes. Record information on Wellness AssessmentForms, in Progress Notes (weekly) and Community Log (as needed)
Maintain regular communication with physician on individual health careissues. When a status change is identified, be the primary liaison with healthcare professional including pharmacy, lab and Hospice etc
Actively participate in monthly care planning meetings, Bi-Annual Care Conferences,Quality Assurance Meetings, Family Clinical Reviews, and Hospice meetings etc
Actively participate in follow-up actions and next steps. Educateas needed. Participate in Monthly Staff Meetings and provide In-service Training as requestedby ED. Based on content, training maybe in partnership with Delegating RN
Coordinate (in conjunction with ED) dietary needs with FSD and environmental needs with the MaintenanceCoordinator
Assist the ED/AD with identifying and resolving employee concerns, complaints,grievances and issues
Assists the ED in completing90-day and annual performance reviews. Whenrequested by ED, will assist in counseling caregivers and medication technician
Available 24/7 to support health and wellness needs of the Residents
Direct the community programs for infection control, quality assurance, outcomes monitoring and other oversightprograms
In partnership with theDelegating RN, review all resident charts on a regular basis, checking for content, clarity,completeness, and appropriateness of care. Responsible
for the quality of all resident charts, including compliance with applicable regulations and thehighest standards of nursing practice.
Instruct caregiving staff on clinical and nursing issues as requestedby the Delegating RN
On day of move-in or within 24 hours, meet with the resident todiscuss: Check-ups, establish baseline, start clinical chart and review/establishcare needs and schedule in coordination with ED/AD/RN
Coordinate services of outside agencies providing health care or otherassistance to residents, including home health agencies, physical therapists,hospice, laboratories, durable medical equipment suppliers and otherprofessional services
Attend classes, seminars and meetings. Maintain a high level of knowledge regarding current nursing theory, trends and regulatorychanges
In the absence of ExecutiveDirector and Assistant Director, the Wellness Coordinator is responsible for the day-to-day operations of the community
Other duties as assigned
Job Qualifications : Must hold acurrent Maryland license as a registered CMT. Must have a minimum of 5 years' experience in the geriatric industry orrelated field. A Bachelor's degree inhealth services is preferred. Requirements: Must be current on all vaccinations including COVID- 19 andreceive CDC recommended booster doses.
