HIV Mental Health Care Coordinator

JOB SUMMARY
The HIV Mental Health Care Coordinator at Rainbow Health is the first point of engagement, providing clinical support and linkage to additional ongoing services and resources for clients eligible for Ryan White Program services for people living with HIV. This role will help clients identify their individualized goals for mental health care and the services needed to match those needs. Outside of providing direct services to clients, the HIV Mental Health Care Coordinator will also engage in community outreach with other organizations and providers.

ESSENTIAL FUNCTIONS
  • Using engagement strategies to develop rapport, build relationships, and provide education around available mental health services.
  • Creating client charts in the EHR system, completing intakes, and managing the waitlist as necessary.
  • Completing diagnostic assessments, clinical measures, and care linkage treatment plans.
  • Providing placement coordination and follow-up to mental health treatment programs.
  • Meeting with clients and providing on-site and on-site services to maximize client recruitment and retention.
  • Coordinating services with stakeholders, including school personnel, HIV service providers, medical professionals, and mental health providers.
  • Providing mental health therapy services to up to 15 clients weekly while coordinating referrals to other providers identified in the treatment linkage care plan.
  • Completing documentation accurately and timely in compliance with agency protocols, contract standards, and legal regulations in multiple systems.
  • Engaging in community outreach to establish and maintain professional working relationships with other organizations and professionals.
  • Providing high-quality mental health services, using evidence-based interventions to Rainbow Health’s targeted populations: people living with HIV, members of the LGBTQ and BIPOC communities,
  • Assisting in the ongoing program evaluation and development by managing and tracking funding options and providing quarterly program and caseload updates to the funder.
  • Acting as a liaison between Rainbow Health management, the community, and other community agencies to develop and maintain successful professional relationships.

MINIMUM QUALIFICATIONS
  • A master's degree or higher in psychology, counseling, or social work.
  • 2 years of experience in providing direct, clinical, mental health services and/or working with mental and substance misuse issues.
  • Licensed/Pre-licensed mental health LPCC, LMFT, LCSW
  • LADC or experience with substance misuse in marginalized populations preferred.
  • Experience with a harm reduction approach.
  • Understanding of the intersectionality of neurodiversity in LGBTQ populations
  • Proficiency in a second language other than English.
  • Individuals with lived experience or a passion towards our target populations (Gay/Bi/MSM/Queer, Transgender, African American, multi-racial communities that are disproportionately impacted by HIV, HIV positive people and those engaging in risk behaviors for HIV in individual and group settings) are encouraged to apply.


WORK ENVIRONMENT

  • Work is currently being done in a hybrid format, splitting time between working from home, at Rainbow Health's office, onsite with community partners, and at outreach events based on need.
  • Employees working in person with clients or out of our offices must show proof of vaccination and attest to their vaccination status upon hire. Employees who are not vaccinated will be required to show a negative COVID-19 PCR test result once a week if working with clients or in our offices. This position is part of a bargaining unit represented by SEIU Healthcare Minnesota & Iowa. All wages, benefits, and other terms and conditions of employment will be established through negotiations with the union.
