Join our Assertive Community Treatment Teams (ACT) in providing comprehensive, integrated care to adults living with mental illness, substance use disorder, and associated medical conditions. Team services are provided where and when clients need them—in their homes, at work, and in other community settings. Team members stay in close contact with clients to help them recover and to respond quickly with more services and supports if circumstances change. 

The Mental Health Practitioner Co-Occurring Disorder Specialist provides treatment services to promote recovery and community integration for persons served on the ACT team, using an Integrated Dual Disorder Treatment (IDDT) model of care. Responsibilities include conducting drug and alcohol assessments and providing stage based individual and group mental health/substance use interventions. With a focus on harm reduction, the Co-Occurring Disorder Specialist provides cross training and guidance to the team on recommended practices to support engagement, active treatment, and maintenance of recovery. Additional job responsibilities include helping people access and maintain benefits and community resources, providing care coordination with external providers, connecting with natural supports, and providing crisis intervention services.

We are hiring our Ramsey County and Washington County ACT Teams. The positions participate in an on-call rotation that entails being available to respond to phone calls after hours for crisis intervention.

Ramsey County: Tues-Sat 8am-4pm. The on-call rotation is every 8-10 weeks.

Washington County: Mon-Fri 8am-4pm (Wed is 10am-6pm). On-call rotation every 4-6 weeks.


Qualifications:

  • Bachelors or Masters Degree in Psychology, Social Work or related field.
  • 2,000 hours experience working with adults with mental illness and substance use disorder.
  • LADC licensure preferred.

Duties and Responsibilities: 

  • Work with the team to provide ACT services for clients, assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the treatment team.
  • Assist in the development of comprehensive assessments, individualized treatment plans, substance abuse treatment, and other tools necessary to assist clients in the community.
  • Assist clients with all aspects of psychiatric rehabilitation and all aspects of obtaining and maintaining housing. Will assist and /or teach clients independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting. Will assist clients with making and keeping appointments and any other tasks that are required through the client’s recovery process.
  • Work with the vocational counselor to encourage competitive employment and assist in fostering positive work skills for clients.
  • Attend commitment and other court hearings as required. Complete required court reports.
  • File reports with Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other mandated/required reports when applicable.
  • Take the lead in assessing, planning and treating substance abuse for assigned clients both individually and in groups.
  • Actively and assertively engage and reach out to clients’ family members/significant others, including landlords and employers after obtaining client permission.
  • Perform shift management duties according to established policies and procedures.
  • Provide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff on a rotating schedule.
  • Available for weekend and holiday duties on a rotating schedule.
  • Attend and participate in all scheduled team meetings.
  • Must transport clients in the community.
  • Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
  • Maintain a pattern of regular work hours.
  • Other duties as assigned.

Competencies:

  • Knowledge of symptoms and course of serious and persistent mental illness, medications, chemical dependency, substance abuse, and other therapies.
  • Knowledge of current principles, practices and techniques of case management.
  • Knowledge of community resources.
  • Be knowledgeable about and utilize various substance abuse treatment modalities such as harm reduction and motivational interviewing.
  • Have knowledge of evidence-based clinical practices.
  • Have demonstrable working knowledge of vulnerable adults laws and other pertinent rules and regulations.
  • Knowledge of supportive counseling techniques, independent living skills/teaching and crisis intervention techniques.
  • Utilizes crisis assessment tools for suicidal and aggressive behavior.
  • Assess and manage crisis situations.
  • Develop appropriate therapeutic relationship with clients.
  • Able to use a computer for word-processing and data base management.
  • Must have good interpersonal skills and ability to work independently and with and be part of an ACT team.
  • Must have a car, driver’s license and be able to drive, day or evening hours.
  • Be able to speak, clearly understand and write the English language.
  • Be able to accommodate flexible working hours.
