Radiant Health is a leading community mental health center dedicated to providing high-quality mental health services to individuals in Grant and Blackford Counties in Indiana. Committed to fostering a culture of compassion and well-being, Radiant Health seeks a dynamic and experienced professional to join its team as Case Coordinator for our Community Based Services team.

As part of the treatment team, the employee in this position provides assessments, crisis intervention, and engages family members in a therapeutic relationship dealing with a variety of problems which could include child management, personal and family adjustments, finances, employment, and emotional/mental impairments. Works closely with community agencies that are involved with the family and attends staff meetings to discuss ongoing treatment for the client and their family. Employees must function in conformance with organizational policies, procedures, and ethical standards as well as local, state, and federal regulations.

MINIMUM QUALIFICATIONS

• BS/BA in mental health field, BSW preferred.
• Competent to create documentation relevant to the treatment plans and assessments, review and update treatment plans, as required.
• Competent to understand the specific communication, developmental and treatment needs of children and adolescents and family systems approach.
• Negative Mantoux.
• Has or be willing to obtain CPR Certification.
• Have and maintain a valid Indiana driver’s license and proof of automobile insurance upon demand. Use personal vehicles to conduct Center treatment needs and business.
• Able to see, hear, speak, read, and communicate.
• Able to work full time.
• Able to use computer for data entry.

Job Type: Full-time

Pay: $16.00 - $17.00 per hour
