Position: Certified Community Health Worker - Bilingual Spanish & English

Supervisor: Community Liberation Program Manager

Work Schedule: Full time, 40 hours per week with flexible hours, with some weeknights and weekends

Location: Southern Oregon (Jackson or Josephine Counties specifically) or Portland Metro Area

Salary: $52,000 - 56,500 PLUS $5,000 Hiring Bonus at 90-days retention

Raíces de Bienestar (Raíces) is a nonprofit in Oregon dedicated to reclaiming the emotional and mental health of Latine communities across the state and beyond. Raíces offers a continuum of Latine culturally grounded wellness programs and services, focused working alongside traditional health workers and community leaders to elevate the strengths and wisdom of the community. We seek a skilled and experienced Latine-identifying bilingual community health worker to join our team and support both our community liberation programs and our clinical services.

Primary Function: This Community Health Worker (CHW) position will support across the three pillars of services for Raíces de Bienestar (Raíces): clinical services, community liberation programs, and capacity building services. Primarily, this position is responsible for coordinating and supporting our community liberation programs as well as the coordination of care for our patients in the Raices’ clinic. This position will have high interaction and visibility with community members who seek mental health services and engage in our programs at Raíces. They will be incredibly competent at supporting participants in navigating systems, eliminating barriers to care, and supporting access to our services and other resources.

Responsibilities:

  • Provide support with planning, promotion, recruitment, registration, facilitation and evaluation of community liberation programs, including but not limited to Charlas, pláticas, healing circles, talking circles, community fairs, tabling events and other outreach and community engagement events and activities
  • Collaborate and coordinate community liberation and engagement activities with other community-based programs, mental health organizations and public entities when needed
  • Follow-up as needed with community liberation program participants to support social and emotional needs, provide resources and connect to other internal programs and community partner services
  • Support capacity building activities, included but not limited to the provision of trainings, presentations and consultation for organizations seeking to improve their care for Latine communities
  • Answer clinic and/or community liberations phone, responding and documenting calls, and coordinating responses accordingly
  • Support prospective patients and patients with coordinated care, service enrollment and engagement. This includes but is not limited to completing eligibility mental health screenings, supporting intake process, helping patients access electronic medical record portal, completing paperwork.
  • Support patient care including scheduling, coordinated care, referrals and case management support

● Complete appropriate and timely documentation in electronic medical record system

● Engage in logistical activities (e.g., printing, organizing, creating, reviewing materials) across the organization

● Maintain records of referrals to our clinic and supports the referral process of prospective clients as needed

● Attend any other needs, requests, projects, committees and provide general support

Qualifications

The ideal candidate will have experience as a community health worker in a community-based setting and have knowledge in mental health systems. In addition, the candidate will have a passion to serve the community in emotional and mental health needs, services and education. The preferred candidate will have taken Raíces de Bienestar’s mental health traditional health curriculum and be a certified community health worker.

Required Knowledge, Skills, and Behaviors:

  • Bi-lingual in English and Spanish is preferred
  • Currently holding a community health worker certification in Oregon
  • Community-focused, and culturally responsive
  • Experienced in popular education modalities and liberation practices
  • Able to work evenings and/or weekends
  • Must have valid driver's license and ability to drive to community events
  • Excellent at interpersonal skills and communication
  • Experienced with technology, including Microsoft suite products.
  • Able to learn and adapt to new technologies
  • Excellence at protecting patient information and HIPAA compliance protocols, standards and procedures
  • Strong time management, meeting deadlines and organizational skills
  • Effective written and verbal communication skills, preferable in Spanish
  • Effectively at identifying and prioritizing issues to respond proactively
  • Strong work ethic and ability to attend to multiple responsibilities simultaneously
  • Strong at maintaining confidentiality regarding information pertaining to all clients, trainees and community members served
  • Knowledge of community resources and agencies in Southern Oregon and/or Portland Metro geographies
  • Assure that all programmatic activities are carried out within cultural, legal, and ethical standards
  • Must be available to travel across the state of Oregon as needed, including some overnight travel
