Responsibility: The hospitalist nurse coordinator collaborates with the hospitalist care team to coordinate and manage the process of patient care from the point of admission to the hospital, through discharge and hand-off to a community care provider.

Reporting Relationship: Director of Operations

Essential Functions:
 Clinical Care Coordination

• Actively participates in daily handoff meeting with hospitalist team to ensure plan of care for patients is communicated at the beginning and end of each shift.
• Monitors patients waiting for admission from the Emergency Room or transfer.
• Communicates pertinent information regarding patient status to the hospitalist for the shift and assists with orders or phone calls as needed.
• Maintains detailed registry of calls / communications to monitor for trends.
• Monitors workflow of team to assist with balance of workload, to expedite timely admissions.

- Organizes the patients pending on a priority list

 Quality Improvement

• Participates in data collection for quality improvement initiatives: conducts clinical audits, collects care delay information.
• Reviews admissions for Quality metric details:

- Core Measure compliance- QEP metric compliance

• Enters CMS quality metric data into Quantum data repository.

 Education/Public Relations

• Educates and promotes hospitalist program among hospital staff

Education, Training, Experience:

• Graduation from an accredited school of nursing; BSN (preferred)
• Licensed to practice as a Registered Nurse (RN) in Georgia
• Adult acute care experience required
• Case management experience (preferred)
• Utilization management experience (preferred)
• Well-developed communication and influence skills

Pay: $85,000.00 - $93,000.00 per year
