Job Title: RN Care Manager
As an RN Case Manager, your role will involve managing patient care and providing services that require substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education. In addition to excellent written/verbal communication, problem-solving, and decision-making abilities, the ideal candidate should demonstrate experience in developing and maintaining rapport with patients and families, as well as the ability to work well with an interdisciplinary team.
Working Conditions:
- Primarily works in the patient's residence.
- Travel to long-term care facilities, hospitals, and area agencies and businesses may be necessary.
- Travel within the entire Good Samaritan Hospice service area will be required.
- Overnight travel outside of the GSH service area may be necessary for continuing education.

- Graduate from an accredited school of nursing.
- Currently licensed to practice nursing by the Virginia State Board of Nursing.
- Two years of experience in acute care, home health, hospice, med/surg, or geriatric nursing.
- Current Virginia driver's license and driving record that meets the standards set forth in current policies.
