MUST:
Experienced Registered Nurse
1 years of experience as a Registered Nurse
1 years of experience as a Registered Nurse working in Primary Care, Adult Medicine, Behavioral Health, Family Medicine, Pediatrics, Infectious Diseases, and/or Obstetrics Gynecology
Strong knowledge of chronic diseases including cardiovascular, diabetes and asthma
Community outreach knowledge and experience working with diverse populations
Comfortable handling patient assessments
Ability to work independently, to supervise and to work in a team-oriented environment
Excellent interpersonal and written communication skills
Must be well organized, flexible and action oriented
Demonstrated ability to effectively work and communicate with diverse populations
Demonstrated proficiency with business software (e.g. Microsoft Office Suite)
CPR certification
Licensed in the District of Columbia as a Registered Nurse
Graduation from an accredited school of nursing with a minimum ADN, BSN preferred
DUTIES:
Assist in the care and management of healthcare conditions
Provide direct nursing care and patient education
Assess patients
Develop treatment plans for patients
Coordinate care and follow up with patients and collaborate with providers
Enter and update patient information in EMR
Delegate duties to other clinical staff members
Handle data collection
