Qualifications and Requirements:

  • Responsible for complete referral process, including identifying home health needs, verification of benefits, and coordination of care. 
  • Cultivate new and existing relationships resulting in company growth.
  • Initiate and expand referral network.
  • Comply with all aspects of the company's compliance and HIPPA regulations.
  • One year of home health, marketing, or case management experience or a combination of each.
  • Must possess a valid driver's license.
