Job Summary:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, conducting assessments, and implementing care plans to ensure optimal patient outcomes.
Responsibilities:
- Conduct patient assessments and develop individualized care plans
- Collaborate with healthcare providers to ensure appropriate utilization management
- Coordinate and monitor patient progress throughout the continuum of care
- Utilize acute care knowledge to address patient needs effectively
- Maintain compliance with HIPAA regulations and other healthcare standards
- Engage in behavioral health interventions as needed
Experience:
- Active Registered Nurse (RN) license in the state of practice
- Minimum of 1 years of experience in home health
- Proficiency in patient assessment techniques and utilization management practices
- Experience in computer charting (Wellsky) a plus
