SUMMARY

The Certified Nursing Assistant (CNA) performs varied and diverse work and is responsible for person-centered service to people with disabilities served by PDI. The CNA has knowledge of and involvement with areas and programs related to Medicaid. The CNA(D) ensures the safety of people served and fellow employees in all operations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Person-Centered Support

  • Gain personal knowledge of and develop relationships with people served
  • Support behavior management for people served
  • Strictly follow safe practices, guidelines, and protocols, especially when providing services including but not limited to medication administration, transfers, personal care, mechanical lift transfers
  • Adhere to strict infection control and prevention policies and best practice guidelines
  • Provide physical support and assistance to people as required
  • Document medical services or treatments provided to people served
  • Daily document concerns or actions taken for people served in a timely and efficient manner to include intervention and follow-up as necessary
  • Assist people served with activities of daily living including but not limited to toileting and toilet hygiene, bathing and showering, dressing, feeding, functional mobility, personal hygiene
  • Complete, or support the completion of, incident reports for all reportable occurrences
  • Engage in skilled interaction with people served to provide safe and proper medical care
  • Support medical management for people served and establish strong and positive mentorship by providing quality support
  • Respond to and follow through with emergencies including but not limited to acting as liaison with emergency and law enforcement personnel, accompanying a person served if he or she is required to visit a hospital or other medical facility, and remaining present until relief appears including but not limited to parent, family member, private provider, case worker
  • Responsible for medication administration and gastronomy tube (“G-tube”) use
  • Adhere to PDI policy regarding confidentiality and the Health Insurance Portability and Accountability Act (“HIPAA”)
  • Accurately administer medication per licensed independent practitioner (“LIP”) and/or primary care physician (“PCP”) orders while adhering to qualified medical medication personnel (“QMAP”) guidelines
  • Accurately document medication administration in a timely and efficient manner
  • Accurately perform blood glucose monitoring and recording with knowledge of critical needing immediate intervention and report to Registered Nurse immediately
  • Safely provide nutrition assistance including but not limited to G-tube feedings as ordered by LIP and/or PCP while accurately following protocols which may include performing by-mouth feedings and/or assistance with food preparation
  • Accurately record and document nutrition and fluid intake as ordered by LIP and/or PCP and/or per individual protocol
  • Accurately perform, read, and document vital signs (temperature, pulse, respirations, blood pressure, and oxygen saturation) and report


Individual Plan Development

  • Ensure proper documentation is included with individual plans (“IP”) to justify and support the provision of services by PDI
  • Support the objectives and goals of each individual support and service plan (“ISSP”) as related to the safety and health of people served

Liaison

  • Professionally and effectively communicate and provide excellent customer service to people served and others including but not limited to family members, private providers, LIPs, PCPs, and guardians of people served and staff and co-workers related to the health and safety of people served
  • Respond to critical issues and crises and report to appropriate staff and Registered Nurse and program supervisor immediately by verbal communication or telephone contract
  • Respond to non-critical or informational issues and report to the Registered Nurse and program supervisor by email communication within one business day
  • When necessary, accompany people served to an off-site emergency department and remain present until relief appears including but not limited to parents, family members, private providers, case worker
  • Initiate and maintain positive and professional service with people served and other parties involved in their life
  • Consistently maintain a helpful and professional manner in dealing with all parties
  • Build community awareness and support for PDI and its programs
  • Responsible for responding to and managing unscheduled visits to people served

Program Assignment

  • The CNA is assigned to one or more specific programs as explained on the attached “Program Assignment Fact Sheet” which is included as part of this Summary Job Description
  • The CNA may be required periodically to perform certain duties to support the routine work of DSP I (D) staff with regard to programs, activities, and services provided to people served

Other

  • Assist with traffic, including directing people served to appropriate classrooms and designated areas
  • Assist during meal and program periods as scheduled and as needed
  • The CNA may be required periodically to perform certain duties supporting the routine work of Direct Support Professionals I and others concerning programs, activities, and services to people served by PDI

Vehicle Use

  • This position does require that you drive a PDI vehicle on PDI business; the position may require you to drive a personal vehicle on PDI business

SUPERVISORY RESPONSIBILITIES - The CNA has no supervisory responsibilities.

MINIMUM REQUIREMENTS FOR ALL PDI POSITIONS

You must be age 21 or over to work at PDI. You must also pass pre-employment and random screens that may occur at any time and without notice while employed by PDI. You must be computer-literate, comfortable with technology, and with well-developed oral and written communication skills. You must be able to complete and pass First Aid, CPR, and other required training.

If the position requires you to drive a PDI vehicle on company business, then you must possess a valid driver’s license and agree to a review of your motor vehicle record for addition to PDI’s authorized drivers list. In addition, if the position authorizes you to drive your vehicle on PDI business then you must provide proof of automobile insurance and proof of required automobile insurance limits.

REQUIRED EDUCATION AND EXPERIENCE

Age 21. High school diploma or general education degree (“GED”) required. The CNA must be a Certified Nurse Assistant (“CNA”) in good standing as credentialed by the Colorado State Board of Nursing. The CNA must have two years of experience working with people with disabilities in a day program or residential program setting or an equivalent combination of education and experience. The CNA(D) will complete all required training including but not limited to cardiopulmonary resuscitation (“CPR”), First Aid, and required periodic refresher courses and continuing education units in the required periods.

LANGUAGE ABILITY

Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence clearly and legibly. Ability to speak effectively with people served and their support networks and co-workers.

MATHEMATICS ABILITY

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.

COMPUTER SKILLS

Knowledge of word processing software, spreadsheet software, and Internet software. Ability to learn and gain proficiency in other software and applications used by PDI.

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several variables.

WORK ENVIRONMENT

The CNA(D) works in the main PDI facility. Reasonable accommodation may be made to enable persons with disabilities to perform essential functions. While performing the duties of this job, the CNA(D) is occasionally exposed to outdoor weather conditions and occasionally exposed to wet or humid conditions (non-weather). The noise level in the work environment is usually moderate but can often be elevated and hectic.

POSITION TYPE AND EXPECTED HOURS OF WORK

This is a non-exempt full-time position based. The hours will generally be 7:30 a.m. until 3:30 p.m. Monday through Friday unless notified by your Superior. Hours may vary based on a variety of factors. An unpaid 30-minute meal period is required.


PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodation may be made to enable persons with disabilities to perform essential functions.

The CNA must regularly lift and /or move up to 50 pounds, frequently lift and/or move up to 100 pounds, and occasionally lift and/or move more than 100 pounds with the aid of an assistive device. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this Job, the CNA(D) is regularly required to reach with hands and arms and talk or hear. The CNA(D) is frequently required to stand; walk; sit; use hands to finger, handle, or feel; climb or balance, and stoop, kneel, crouch, or crawl. The CNA(D) is occasionally required to taste or smell.

TRAVEL

The CNA has no out-of-town travel-related responsibilities but may have limited required local travel
