SHERO Shaping Healthy Equitable Reproductive Outcomes program integrates medicine, public health and prioritizes technology to improve the health, lives and wellbeing of moms-to-be and their babies.

SHERO’s vision is to advance equity by eliminating the Black maternal & infant mortality / morbidity crisis across the US.

The primary focus of this role is to provide support and guidance to pregnant individuals and new parents in the community.

As a Perinatal Community Health Worker, you will play a crucial role in promoting maternal and infant health, offering educational resources, and connecting expectant and post-partum moms with essential perinatal education, services and support.

This is a grant-funded, full time, benefitted offer prenatal and social support throughout and beyond pregnancy using a virtual outreach model.

The CHW position is grant funded and renewed annually based on performance and funding availability. Employment is provided by Heluna Health.

The salary for this position is $17.31 per hour. Interested applicants must submit a cover letter and resume. Must be a resident of New Orleans, Louisiana to qualify for this position.

This position is remote and must be available during normal Pacific Time Zone working hours. This position will require travel up to 10% or more.

May be required to work flexible hours. If hired for this position, applicant will be required to provide proof of full vaccination for COVID-19 prior to the start date;

or have a valid religious or medical reason qualifying the applicant for an exemption (that may or may not require accommodation).

The applicant will need to have the qualifying exemption. ESSENTIAL FUNCTIONS

  • Be a self-motivated team-player with excellent communication and interpersonal skills.
  • Work closely with and report to the SHERO Director
  • Work from home and directly in the community.
  • Work flexible hours, including some evenings and weekends for community events.
  • Have reliable internet access and transportation. Community Outreach :
  • Engage with community members to identify pregnant persons in need of support.
  • Co-support and meet the needs of SHERO participants (moms and babies).
  • Conduct SHERO outreach programs and deliver presentations to raise awareness about perinatal health and available resources to key stakeholders.
  • Collaborate with healthcare providers and community organizations to ensure a comprehensive support network including WIC, Snap, Section 8, Healthy Start, Medicaid, etc.

Perinatal Education, Support & Advocacy :

  • Provide information on perinatal and postpartum care, breastfeeding, nutrition, infant care and more.
  • Offer emotional and social support to individuals and families during and after the perinatal period.
  • Assist pregnant persons in accessing healthcare, social services and resources.
  • Increase awareness of SHERO on social media and other outlets. Home Visitation :
  • Conduct home visits, as assigned, to assess the needs of pregnant persons and new parents.
  • Offer personalized guidance on creating a safe and healthy home environment for infants. Advocacy :
  • Advocate for the rights and well-being of pregnant individuals and families within the community.
  • Collaborate with local organizations to address systemic issues affecting perinatal health. Data Collection and Reporting :
  • Maintain accurate records of interactions and services provided.
  • Compile data for program evaluation and improvement.
  • Helping Black moms to be access health resources.
  • CHWs support the need of local pods on the West and East Banks
  • Facilitating communication and client empowerment in interactions with health care / social service systems
  • Participate in program at community meetings, outreach events, health fairs, webinars, etc.

  • Helping Moms understand their health condition(s) and develop strategies to improve their health and well-being.
  • Helping to build understanding and social capital to support healthier behaviors and lifestyle choices.

  • Linking Moms to health care / social service resources
  • Providing informal counseling, support and follow-up as needed with SHERO Moms

  • Ability to provide clear and concise instructions. EDUCATION / EXPERIENCE
  • Preference is given to extraverts with a high school diploma or equivalent, additional education or certification in a relevant field.
  • Proven experience in perinatal community outreach or health education.
  • Empathetic and non-judgmental attitude towards diverse populations.
  • Knowledge of local healthcare and social service resources.
  • Ability to work independently and as part of a collaborative team.
  • Proficient in Microsoft Office 365 applications (Outlook, PowerPoint, Word, Excel).
  • Proficient in conducting Zoom and other virtual platform meetings and webinars.
  • Proficient using computers, tablets, printers, cellphones, Wi-Fi and Bluetooth.
  • Proficient in time management, multi-tasking, organization and meeting deadlines.
  • Experience with Apple products and services (I-chat, Facetime, I-Message)
  • Experience posting on social media platforms (Instagram, YouTube, Facebook) Certificates / Licenses / Clearances
  • A valid Louisiana Driver License or the ability to utilize an alternative method of transportation to carry out job-related essential functions.
